Medical Release Form
Every student is required to have an updated Medical Release form to be involved in our events for 5th grade through High School.  Our most up to date form is available by following the link.  This form not only includes the medical information needed for each student, it also now includes the option for each family to select the way they receive information about upcoming events and ministry opportunities.  Please take a few minutes to fill this form out which will be good through the 2011-2012 school year.
